Please complete our short form below to request Online Access if you are not already signed up to use the NHS App. Your completed form will be emailed to the Practice, you will also receive a copy.

Proxy Access Form

Proxy access allows a close friend/relative etc to access parts of your online account with their own login in order to assist you to book appointments, order medication etc. This usually requires the consent of both parties. Parents can apply for proxy access to their child’s record up to their 11th birthday. All applications will need to be submitted with a suitable ID and will be carefully considered by the practice on an individual basis. Please download and complete the form, Proxy Access. Bring the completed form, along with suitable ID, to the Surgery.
