Using the Self Referrals

All of the services below can be used with a self-referral if you meet the criteria listed. If you do not meet the criteria or do not think you are eligible for self-referral, then you may still be eligible to use the service but would need to discuss with a Doctor your circumstances – in this case please raise an eConsult with your details.

Adult Social Care Direct is the main contact point if you want to talk about any care needs you may have. The phone service is open 24 hours a day, seven days a week. You can find a lot of information about adult social care on our website, which means you may not have to contact us directly to get the answers you need.

Crisis Resolution and Home Treatment Team

The Crisis Resolution and Home Treatment Team is a team of experienced mental health staff, which includes nurses, social workers, psychiatrists and pharmacy staff. We offer assessment and home treatment for people over 16 experiencing a mental health crisis, as an alternative to hospital admission.

How to get referred

Telephone referrals from Care Co-ordinator, GP referral, Primary Care Mental Health Team, Self referral and Specialist Mental Health Service is accepted
